Across the country, three in four adults are living with at least one chronic condition. Elevance Health is helping people manage chronic conditions by creating more personalized, connected experiences that make it easier to understand care, stay on track, and know what to do next.

Elevance Health is working to simplify that experience by using data, digital tools, and proactive outreach to connect the people it serves to the support they need when they need it, through services offered through its health plans or its healthcare services business, Carelon.

“Managing a chronic condition can be challenging, especially when care feels fragmented or difficult to navigate,” said Shantanu Agrawal, MD, Chief Health Officer at Elevance Health. “By combining data-driven insights with more personalized, proactive support, we can help people feel more confident in their care, stay engaged, and take steps earlier to support their long-term health."

One example is Proactive Member Engagement (PME), an enterprise-wide program that brings together medical, pharmacy, behavioral, and social data to anticipate members’ needs and identify gaps in care. Using advanced analytics, PME delivers timely, personalized outreach through preferred channels like text, phone, and email, helping members take action without navigating fragmented communications. Operating at scale with more than 12 million interactions, PME creates a more coordinated, “one member, one experience” approach that supports increased wellness visits, improved medication adherence, and better overall outcomes.

For members with more complex needs, Concierge Care provides ongoing, personalized support to make managing a condition more manageable day to day. Members are connected to a dedicated care team that helps build tailored care plans based on their needs, while offering tools to track symptoms, access education, and connect with care professionals when questions arise. The program emphasizes proactive, ongoing engagement, with regular check-ins and 24/7 nurse support to help members manage side effects, adjust care plans, and make informed decisions. By combining real-time insights with continuous support, Concierge Care helps members stay on track with treatment, reduce avoidable emergency room visits and hospitalizations, and feel more confident managing their health.

On the digital front, Sydney Health extends this experience by delivering personalized, real-time guidance directly to members. By using information about a health plan member’s benefits, care history, and current health needs, Sydney Health surfaces tailored nudges and recommended next steps to help individuals schedule appointments through Find Care, complete screenings, and take simple actions to manage their conditions. It presents this information in a clear, easy-to-understand format with guided prompts and reminders, helping translate complex care plans into actionable steps that support ongoing engagement and more confident decision-making.

Elevance Health is introducing Total Movement Care (TMC), a new musculoskeletal (MSK) care experience designed to connect members to the right care earlier in their journey. TMC offers access to MSK-trained clinicians, virtual-first physical therapy, and guided navigation to appropriate in-network specialists when needed. Initially available to a subset of members in its affiliated fully insured health plans, the program is expected to expand over time. By enabling earlier assessment and more coordinated, evidence-based care, TMC helps improve outcomes, reduce unnecessary procedures, and create a more streamlined experience for members.
